Professional referrals
All referrals to LHT are required to have the following minimum requirements of information- a completed LHT Referral Application Form, an up to date list of previous convictions, a complete and up to date OASys risk assessment and the most recent Pre Sentence Report or Parole Assessment Report. This requirement is supported by Probation Circular 07/2007 and LHT’s Service Level Agreement with the Ministry of Justice.
- Aims and objectives of the Langley House Trust (LHT). The LHT exists to help men and women who have offended or who are at risk of offending, to remain crime free, to reduce their dependence on harmful substances, to manage conditions which adversely affect their behaviour and to improve their individual potential in a range of areas, so that all service users become welcome contributors in their communities. The LHT offers a wide range of residential and support services nationally to men and women aged 18 years and over.
- Range of residential and support/ care services. Not every service will be suitable for your applicant. Services are varied in terms of their geographical location, the client group that they seek to serve, the staffing levels and security arrangements in place and the local referral arrangements/ exclusions in place with probation, police, mental health services and local communities. All project information leaflets can be accessed on the website. LHT’s National Referral Protocol and many of the local referral protocols can also be accessed in this way.
- Referral documentation. All referrals to LHT are required to have the following minimum requirements of information- a completed LHT Referral Application Form, an up to date list of previous convictions, a complete and up to date OASys risk assessment and the most recent Pre Sentence Report or Parole Assessment Report. Receipt of these documents invariably prompts requests for further information, particularly for harder to place individuals. LHT has a secure network- Criminal Justice Secure Mailing. - The Central Referrals Team can be contacted on 01924 339413. We suggest you contact CRO if your client does not have the minimum documentation requirements, as they will need to give approval to proceed with your referral. Staff at Central Referrals are best placed to discuss the range of projects and services available, particularly if your applicant has no local connection, cannot return to is/her home area or has complex needs and is proving difficult to place. Once we have the referral documentation, Central Referral staff will recommend the most suitable LHT project/service and will allocate the referral with your approval.
- Referrals at projects. Once all of the referral documentation has been received at project level, project staff will make assessment arrangements and will keep you informed as to the progress of your referral. You too can contact projects directly to track the progress of your referral.
- Funding. LHT receives funding from a variety of sources including The Ministry of Justice, Supporting People funding from Local Authorities, rental income from service users and Housing Benefit and charitable donations. For some applicants they require a registered care home placement and /or extra care/support funding for additional dedicated staffing/ services needed in order to manage particularly challenging or vulnerable individuals. Most of these applicants will require funding following a Community Care Assessment and funding will come via local Health and Social Care Teams, local Drugs Intervention teams and/or from CMHT’s via local Primary Care Trusts .
